Assessment data is the results or reports of the assessment that are done to the students. Which are then used to adjust teaching.
The data that the students were assess over was their fluency test. The students are at their of their school year and are suppose to be reading at least 90 words per minute.

Student 1: 45 WPM Student 2: 50 WPM
Student 3: 56 WPM
One strategies that I have implemented is that the students take home a folder with different passages. They have to read one story per week at least twice at home and the parent has to time them and write down the information. At the end of the week the students are given a fluency test over that one passage.
The second strategy is that I put these students in small group and allow them to choral read.
