Membranoprliferative Glomerulonephritis
Nephrotic Syndrome
Type 3
immune and complex,
subendothelial and subepithelial space
Type 1
most common
circulating immune complexes (hep b/c). complement activation
OR
2. alternative complement pathway (c3 convertase) mutation/autoantibody which modifies proteins that regulate this process. IgG (Nephritic Factor) may bind to C3 convertase - more stable. no immune complexes just complement
either the immune complexes (1) or just complement disposits(2) end up in the
subendothelial layer.
inflammtory cells recruited and destruction, thickened basment membrane, mesangial cells proliferate (mesangial interposition), splits basement membrane
train track appearance on light microscopy
granular on immunofluorense
Type2
complement deposits, no immune complexes
IgG, nephritic factor, deposits complements in
basement membrane
(not subendothelium, no tram track abnormality)
also present as nephritic - inflammation:
hypertension
oliguria
Haematuria
azotemia
Tx: steroids (inconsistent response)
Dense Deposit Disease
