Levodopa/Carbidopa (Sinemet)
Levodopa is a dopamine precursor
Carbidopa helps it cross the BBB
Adverse Effects
Nausea and vomiting
From stimulation of dopamine receptors at the chemoreceptors trigger zone
Tardive Dyskinesias (involuntary movements: bobbing, tics, choreiform movements)
Due to hyperactivity
Psychosis
Orthostatic hypotension
Dark sweat and urine
Restore levels of ACh and dopamine so decrease in tremors
first line drug for Parkinson's disease
Interactions
MAOI inhibit breakdown of dopamine
Hypertensive crisis
High protein diet will compete with levodopa for absorption
Will decrease absorption into gut
"Off times"
Increase levels of dopamine
Selegiline (Eldepryl)
MAO-B inhibitors
Inhibits break down of dopamine
Adverse reactions
Insomnia
Due to CNS excitation from dopamine
Orthostatic hypotentsion

Interaction with tyramine!
Hypertensive crisis due to increased levels of dopamine
Tyramine secretes dopamine and is also metabolized by MAO
Benztropine (Cogentin)
Centrally acting anticholinergics
Block ACh at muscarinic receptors (NO guy by the pool)
Improves balance between dopamine and ACh
Reduces tremor
By decreasing levels of ACh
Adverse Effects
Dry mouth
Constipation
Urinary retention
Blurred Vision
Tachycardia
Entacapone (COMTan)
COMT Inhibitor
Inhibiting COMT increases concentration of Levodopa and thus Dopamine
Adverse Effects
Same as levodopa
