We're going with the assumption that if a page is stored on disk the PFN is the disk address instead.
When the OS receives a page fault it looks in the PTE to find the address and issues the request to disk to fetch the page into memory.
Update the page table to reflect that the page is now in memory and record the physical address in the PFN.
Attempt the INS again and if a TLB MISS occurs replace the value in the TLB and repeat the INS.

Reasoning
In order to support large virtual address spaces that are greater than the size of physical memory we need to make use of disk space.
