PATIENT CARE IN RADIOGRAPHY
PATIENT'S RIGHT
expect all communications & records pertaining his care should be confidential
obtain information as any relationship of his hospital to other health-care & educational instituitions
refuse treatment
refuse to participate in research project
receive informed consent prior to start any special procedure
to examine & receive an explanation of his bill regardless of source of payment
obtain complete current information about his diagnosis, treatment & prognosis
know the hospital rules & regulations apply to his conduct as patient
to respectful care
IDENTIFYING THE PATIENT
call the patient's name
ask patient to repeat his/her name
identify patient's name, age,region to be examined
ask the patient about the region to be examined or the part that hurt/pain to ensure the requisition form is correct with patient condition
read the requisition form
CARE OF PATIENT'S BELONGING
all items must be tagged & placed in the designated safety area by the radiographer
radiographer should not place value on patient's belonging
envelope @ container should be offered to patient to accomodate necklaces, rings & watch
every belonging should be treated with care
must explain to patient about what must be done with personal items in order to keep them safe
DRESSING & UNDRESSING PATIENT
WOMAN'S DRESSED
draw sheet over pt, help her remove brassiere
help her to put the lab gown, then remove the sheet
DISABLED PATIENT
cover pt with draw sheet, lab gown is put to pt, explain to pt what have to be do, ask pt to help
if pt paralyze @ unconcious, ask for help
remove cloth from less affected side to the more affected side
place the gown on unaffected/affected side, pt is covered with draw sheet
tie the gown at the back
LOWER EXTREMITY DISABLED
cloth should be removed from the upper part first
put the lab gown to pt & instruct him/her to loosen belt buckled, buttons @ hooks around the waist & slip his/her trousers down to hips
if cant, reach under his gown & pull the trousers down to hips
PATIENT WITH IV INFUSIONS
place the sleeve on affected side, first, let the bottle fluid passed through, then the tubing onto the arm with the iv infusion in place
if pt cloth have to changed, slip the cloth from unaffected side first
hang the bottle back after complete dressing the pt
YOUNG PATIENT
briefly explain to parent how the pt need to dressed
more relaxed when being accompanied by parent /familiar adult
IMMOBILIZERS
use of immobilizers
any manual method @ physical @ mechanical device, material @ equipment attached to body pt that restrain the movement of the pt & cannot be remove easily
the EQUIPMENT's
safety straps/side rails
used on beds/stretchers - PREVENT falls when pt are unconscious/asleep/weak/sedated
used on radiographic table during waiting periods as precaution to avoid falling
sandbags/sponges
support pt body part such as abdomen, knee, head when lying down - KEEP pt comfortable
put the things under pt body
limb holders/four points restrain
ankle/wrist immobilizers
waist immobilizers
keeps pt safe on the tabletop/bed
RULE FOR APPLICATION OF IMMOBILIZERS
strap must not be tighter as the pt keep moving
immobilizers must be easy to remove quickly
normal anatomy position must maintained
circulation @ respiration must not be impaired
area of immobilized must be padded to prevent injury to skin
if leg immobilizers are applied, used the wrist immobilizers as well to prevent pt from unfasten the device
pt must be allowed mobility as much safely as possible
SUPPORT & PADDING
WHY???
used when pt lying on hard surface
help reduce anxiety/apprehension
pt feel comfortable
bolster under head & knee
releases lumbosacral stress by straightening lumbar curve
relieve strain on the abomen
padding under pt body for immobilize pt
avoid bedsores/decubitus ulcer (heels,sacrum,mid thoracic curvature)
HOW???
put pillow @ cushion under pt head
bolster put under the knee
usage of sponges to allow air flow/prevent wet
MOVING & TRANSFERRING THE PT's
FIRST STEP: must assess the pt's ability to aid in process
identify abnormalities in pt's body alignment
if any, determines the reasons of the problem
muscle damage/dysfunction of NS
malnutrition
trauma
fatique
poor posture
emotional disturbances
support the affected limbs/body parts with pillow, supports blocks/sandbags
assess the mobility of pt's to walk
CARE of SEVERELY ILL PTs on trolley & wheelchairs
pt must be covered with a sheet as much as possible
confused,disoriented/unconscious pt & child cant never be left alone, on tabletop or wheelchairs
pt unstable in sitting cant be left on wheelchair without support
special cares when changing dress of pts with iv canulas & drips
pay attention to pts with various drainage tubes
