Diagnostics
PFT
measure lung volumes and airflow to diagnose pulmonary disease, monitor disease progression, evaluate disability, and assess response to bronchodilators
Skin Test

ensure intradermal, not subcut
properly mark and instruct pt not to wash off markings
refrain from OTC use specifically to alleviate any allergic reaction
accurately document test sites, especially with multiple antigen testing

Sputum
observe the sputum for color, blood, volume, and viscosity
obtained by expectoration, tracheal suction, bronchoscopy, sputum induction
Lung Biopsy
(2) percutaneously or via transthoracic needle aspiration (TTNA)— needle through the chest wall by CT guidance followed by CXray because pneumothorax risk
3) video-assisted thoracic surgery (VATS)—less invasive version of open lung biopsy to view lesions and take biopsy specimens; followed by chest tube placement until lung expands

2. Auscultaion
Fine crackles
Series of short-duration, discontinuous, high-pitched sounds heard just before the end of inspiration.
bronchioles suddenly snap open. Similar sound to that made by rolling hair between fingers just behind ear

Rhonchi
Continuous rumbling, snoring, or rattling sounds from obstruction of large airways with secretions.

Coarse crackles
Series of long-duration, discontinuous, low-pitched sounds caused by air passing through airway intermittently occluded by mucus, unstable bronchial wall, or fold of mucosa.
Evident on inspiration and, at times, expiration. Similar sound to blowing through
straw under water. Increase in bubbling quality with more fluid.

Note the pitch (e.g., high, low), duration of sound, and presence of adventitious or abnormal sounds
3. Percussion
Tympany
drumlike, loud, empty quality

Dull
-
Means: “mixed” solid, fluid-filled, pneumonia
Hyperresonance
Loud, lower-pitched sound
Means: hyperinflated lungs, COPD, acute asthma
Flat
Soft, high-pitched sound of short duration
Means: dense tissue w/o air, posterior chest below level of diaphragm
