- - - - Ketogenic diet (KD) is an effective therapy for
        pharmacoresistant epilepsy: more than half of the patients show a greater
        than 50% reduction in seizures
      - Ketogenic alternatives
        
        Diets containing large proportions of MCTs (usually provided by
        supplementing with MCT oil) are also easier to prepare, more
        palatable, better tolerated, and require less carbohydrate and protein
        restriction than standard ketogenic diets
        • The ketogenic diet has caused a number of adverse effects, some
        serious. Initiation of the diet can result in vomiting, hypoglycaemia, or
        dehydration
        • A modified Atkins diet is often easier to adhere to and can give
        similar benefits to a standard ketogenic
        • Ketogenic diets should always be supervised by a registered
        dietician
    - - Vitamin B6 deficiency has been found in a high proportion of patients with epilepsy
        • Vitamin B6 supplementation has been reported to be beneficial in some, but not all, studies of
        patients with non-vitamin B6-dependent epilepsy
        • While most patients with vitamin B6-dependent seizures can be effectively treated with pyridoxine,
        some patients have only responded to PLP, the biologically active form of vitamin B6, which may
        be due to genetic polymorphisms in enzymes
    - - Magnesium concentrations in serum and cerebrospinal fluid (CSF) were significantly lower in 40
        patients with grand mal epilepsy than in controls. Serum and CSF magnesium levels fell with
        increasing duration and frequency of seizures
        • Oral administration of magnesium has been associated in some cases with an improvement in
        EEG findings and a reduction in seizure frequency
    - - Erythrocyte or plasma Vitamin E concentrations were lower in
        children with epilepsy than in healthy controls
        • In some studies, vitamin E supplementation reduced seizure
        frequency, although no improvement was seen in other studies
    - - Taurine acts as a modulator of membrane excitability in the central nervous system by inhibiting
        the release of other neurotransmitters and decreasing mitochondrial release of calcium
      - Taurine has been administered orally or intravenously at a wide range of doses (200 mg/day to
        21 g/day) for varying periods of time to patients with severe, intractable epilepsy. In some studies,
        a significant reduction in seizure frequency was observed
    - - Remove trigger foods- excessive alcohol, MSG, aspartame
        • Regulate blood glucose and insulin levels through appropriate dietary therapy
        • Consider a ketogenic or modified Atkins diets depending on severity of epilepsy
        • Address any GABA deficiency- with precursors and co-factors
        • Consider B6 supplementation- but not with certain medications
        • Consider taurine or magnesium to stabilise cell membranes
        • Support mitochondrial energy function to overcome ischemic damage problems (see stress
        lecture)
        • Always work in conjunction with the consulting neurologist
        • Be extra careful about drug/ nutrient interactions
        • Increase folate rich foods
        • Increase intracellular glutathione concentrations

        alcoholism, nocturnal myoclonus, depression

        awaken someone– follow a low GL diet and insure that the last meal
        is slow release to maintain consistent blood glucose

        production . Glass warm cow’s milk (if tolerable) contains tryptophan.
        Also ensuring sunlight on the eyes during the day, then darkness at
        night
    - - Iron deficiency, folate deficiency, Vitamin E
        and Magnesium as an anti-spasmodic and vasodilator nutrient to
        improve circulation. Reduce pro-inflammatory foods and help to
        regulate mitochondrial function
  - - - Avoid midday naps – increase activity with regular exercise but not before bedtime
        • Darken the room by drawing curtains – reducing any light entering the room as excess light at
        night will reduce melatonin production
        • Treat underlying cause – depression, phobias, fear, obsessive thoughts. (flower essences,
        homeopathy)
        • Maintain neurotransmitter levels- with nutritional precursors and co-factors
        • Avoid any dietary stimulants after mid-afternoon – caffeine, refined sugars etc. Also avoid heavy
        meals late at night
        • Relaxation techniques – deep breathing exercises, relaxation tapes, reading, journal writing
        • Sleep apnoea – needs medical investigation. Reduce salicylates and maintain ideal body weigh
