Polycythemia Vera (PV)
Medication :star: Regimen for PV :star:
Low dose aspirin/anticoagulants - to prevent/decrease clot formation
Allopurinol (ZYLOPRIM)- reduces gouty attacks
Alpha- interferon (A-IFN) – used for women in childbearing age w/intractable pruritus
paroxetine- for erythromelalgia (dilation of blood vessels in peripheri)
Ruxolitinib (JAKAFI)- inhibits the expression of JAK2 mutation
(Used for pts w/polycythemia-related myelofibrosis)
Anagrelide (AGRYLIN) – Reduc the platelet count & inhibits platelet aggregation
Myelosuppressive agents:
hydroxyurea/hydroxicarbamide
bhlorambucil
busulfan (Myleran)
Health Promotion
-Drink at least 3 L of liquids each day
-Avoid tight or constrictive clothing, especially garters and girdles.
-Wear gloves when outdoors in temperatures lower than 50F (10 C).
-Contact physician at first sign of infection.
-Take anticoagulants as prescribed
-Wear support hose or stockings while you are awake
-Elevate feet whenever you are seated.
-Exercise slowly with advice from physician.
-Stop activity at first sign of chest pain.
-Use electric shaver
-Use soft toothbrush to brush your teeth (do not floss!)

Goal is to reduce blood volume and viscosity and bone marrow activity
-Phlebotomy is the mainstay of treatment. Goal is to reduce Hct and keep it <45%-48%.
-300 to 500mL of blood may be removed every day
-May become deficient in iron but should avoid iron supplementation.
-Hydration therapy is used to reduce blood's viscosity
-Bone marrow suppression
-
Chronic myeloproliferation, leading to increased RBC, WBC, Plt due to more than one DNA mutation
