Wrong patient Ticket Token

Not recognized by BE

Ticket token not encoded

encodign method was deleted

the whole method was obsolete and was deleted with no change indicated

application was working because ticektoken was kept in cache

delete cache prior to commit

review didn't catch the change

just one reviewer checked it

review done in hurry

too many items for patch on last moment

too many reviews for other components

patch was more a new release than patch

lot's of new functionalities

PO wanted this due to high priority

tech lead agreed even if didn't like this before

team has accepted

nobody pointed out that this might be difficult to achive

last time updates for certification

was discovered but just on one practice

was not taken as an issue

didn't know how the functionality is expected to work

this part was thought to be out of our scope

it was confirmed on daily scrum

never tested by the team

the functionality was working for just 1 special patient on DEV3

PSS was working on PROD only

not enough testing patients for each evironment

get patients to test on other envs

was not discovered by QA

this was not explicitly tested - see "was not taken as an issue"

no automation in PI for this functionality

was not in PI scope

was thought to be covered by APTS team

not enough capacity

2 testers were removed fro the team

add new qa

devs help with automation

not able to implement automation for new features

new feature which was brought in in last 14 days

new features were maually tested in PD week instead of working on automation

do not work on implementation in PD week

keep PD week for automation maintenance and env setup

automation in appointments project was not known

functionality was broken by other already existing defects

issues were ignored as being present on lower ENV only

missing E2E testing with appointments team

teams not in touch

think of E2E testing when grooming
