SHOCK :zap:
CARDIOGENIC SHOCK
Occurs when the heart is unable to circulate sufficent blood to maintain adequate peripheral oxygen delivery.
TREATMENT:
Low dose dopamine
HEMODYNAMIC
PARAMETERS
Means arterial pressure
HR :arrow_up:or
Central venous pressure :Varies
CO
Peripheral vascular resistance
pH
Pao2
paCO2
CHARACTERISTICS
-Poor pump performance (heart)
-Low blood pressure
-Crackles in lung sounds
-Pulse rate can be up, down and stable
Caused by extensive infarction of left ventricle, diffuse ischemia, or decompensated congestive heart failure resulting in primary pump failure.
HYPOVOLEMIC SHOCK
-
OBSTRUCTIVE SHOCK
-
CHARACTERISTICS
-Low blood pressure
-cool, clammy, pale skin
-rapid or weak pulse
-rapid or shallow breathing
-cardiac arrest
-lack of O2 to tissures
HEMODYNAMIC
PARAMETERS
CARDIAC TAMPONADE:
Pulmonary artery occlusion pressure
CO
Systemic vascular resistance
PULMONARY EMBOLUS:
Pulmonary artery occlusion  or normal
CO
Systemic vascular resistance
TREATMENT:
IV fluids
DISTRIBUTIVE SHOCK
-
3 COMMON TYPES
ANAPHYLACTIC SHOCK:Occurs when a person reacts severely to a substance to which he or she has been sensitized.
CHARACTERISTICS
-Low blood pressure
-decreased pulse pressure
-flushed, warm, moist skin
-lungs may have wheezes or may be decreased breathing with no sounds
HEMODYNAMIC PARAMETERS:
CO
Central venous pressure
Pulmonary artery pressure
TREATMENT:
Epinepherine
diphenhydramine
salbutamol
ipratropium
corticosteroids
SEPTIC SHOCK:Occurs as a result of widespread infection, usually due to gram negative bacterial organisms, gram positive bacteria, fungi, and viruses.
CHARACTERISTICS
-Low blood pressure
-increased pulse
-flushed or pale, hot or cool, moist skin
-crackle sounds in lungs (if in pulmonary region
HEMODYNAMIC PARAMETERS:
Means arterial pressure
HR
Central venous pressure
CO :arrow_up:then
Peripheral vascular resistance
pH
Pao2
PaCO2
TREATMENT:
IV fluids
Norepinepherine
High dosed dopamine
NEUROGENIC SHOCK:Usually results from spinal cord injury resulting in loss of normal sympathetic nervous system tone and vasodilation.Muscle walls of blood vessels are cut off from nerve impulses that caused them to contract
CHARACTERISTICS
-vasodilation of blood vessles
-maldistribution of blood
-low peripheral resistance
-low blood pressure
-decreased pulse
-flushed, dry, warm skin
-clear lung sounds
HEMODYNAMIC PARAMETERS:
Means arterial pressure
HR
Central venous pressure
CO
Peripheral vascular resistance
pH
Pao2
PaCO2 : increased and decreased rate
TREATMENT:
IV fluids
Atropine
high dosed dopamine
norepinepherine
