Three Types of Anemia
Blood Loss (REGEN): due to BV damage; hemostatic abnormalities (thrombocytopenia); parasitism (hookworms ie); iatrogenic
-
Systemic response: tissue hypoxemia leads to ^^ EPO production; spleen contracts to release RBCs/platelets
Within hours-days; plasma redistributes from interstitial space to vascular space to dilute remaining RBCs (LOW RBC AND PROTEINS)
-
Hemolysis (REGEN): intra/extravascular RBC destruction (RBC parasitism); oxidative injury; immune mediated; severe hypophosphatemia
-
Extravascular: RBC destruction occurs outside of BVs via phagocytosis in tissues--> hyperbilirubinemia, bilirubinuria
Intravascular: RBC breakdown happens inside vessel; hemoglobinuria, hemoglobinemia; hyperbilirubinemia; bilirubinuria
Oxidative Injury: can suspect if presence of HEINZ bodies (red noses on the surface of an RBC--> denatured hemo); eccentrocytes (handbag membrane); ghost cells (IVH)

Causes of Anemia:
-
-
-
-
-
-
BUN/Creatinine: nonregen anemia +azotemia +isosthenuria (USG 1.008-1.012) = LOW EPO due to renal failure
-
RBC Indices
-
-
Characteristic Patterns
Macrocytic and hypochromic: erythroid degeneration, esp with reticulocytosis
