PERSONAL PROFILE:
- 70 years old female, married
- Uncontrolled diabetes mellitus
- Hypertension
- No previous hospitalization
ASSESSMENT:
- Height: 152 cm
- Weight: 64 kg
- recent loss of weight
- Vital signs: temperature- 36.9C, pulse-100, respiration- 26, b/p- 178/84
- peripheral pulse-present
- restless, mild confusion
- poor understanding of disease processes
- inconsistent intake of medication/s
- general malaise, weakness noted
- bilateral feet edema
