CLEARING
Above - neurological, eg. nerve impingement,
Below - Mechanical
OBJECTIVE
Bony abnormalities, Colour changes Wastage(muscles), Swelling
AX posture
lateral plumb line
anterior - symmetry - ear lobes, clavicles, iliac crests, greater trochanters, tibial tuberosity, medial malleoli
Posterior - symmetry - ear lobes, acromion, inferior angle of scapula, iliac crest, PSIS, posterior tibiofemoral joint line, medial malleoli
look at different postures - scoliosis, kyphosis, lordosis, kypho-lordotic, swayback, flat back, anterior/posterior tilt of hips
PROM + OP
passively taking patient through all range of movement at the joint
assessing for ROM, end feels (SHE SHE), pain
Soft, Hard, Elastic - normal
Springy block, Hard when shouldn't, Empty - abnormal
HISTORY
Location, Onset, Sin, Tissue type
Onset - insidious/repetitve/overuse - Tendinopathy - Impingement - Degeneration - Entrapment of nerves (pririformis/discs/foramen etc) - Chronic Pain - Central Censitisation
Onset - Acute/Traumatic - Fractures - Strains or Tears (Muscles/Ligaments/Meniscus/Labrum etc) - Dislocation - Slipped disc (Herniated)
PALPATION
Temperature, swelling, scar tissue, location
AROM + OP
patient performs all active movement available at joint, then apply overpressure, taking movement to end of range
assessing - quality of movement, reproduction of pain, avaliable ROM, willingness to move
ACTIVE RESISTED
testing the different tissues of the area
contractile
- muscles, tendons, musculotendinous junction, tenoosseous junction
non-contractile
- bone, ligament, intervertebral disc, cartilage
