INTERVENTION STRATEGIES

Feeding
For promoting a pincer grip for self-feeding purposes, painting on a piece of paper. Using brush strokes on a vertical surface that will naturally force Brian's hand to flex at the wrist which then automatically puts the forefinger and thumb into position for pincer grip (School Sparks, n.d.).

For promoting a pincer grip for self-feeding purposes, utilizing a sensory bin with edible objects will help to increase his fine motor skills as well as engagement in the activity. This intervention can also help with increasing familiarity with various textures and promoting tactile sensations.
Social Participation
Environmental modifications such as placing Brian's seat closer to his family members at the table during dinner can be implemented to promote opportunities of social engagement.
OT will educate Brian's caregivers of the positive effects of placing Brian in prone position during play time. While Brian is in prone, OT will instruct games to family members incorporated for playtime with Brian. This tummy time play method will be implemented to target Brian's torticollis as well as promoting social engagement (Wittmeier, & Mulder, 2017). The stretching and strengthening of his neck muscles can help with swallowing mechanisms (Nilesh & Mukherji, 2013).
Swallowing/Eating
A distraction strategy will be devised when introducing new textures to Brian's diet, such as using a toy or the caregiver singing while new food is being introduced, to get past his hyper-reaction to the new sensations (Boles, 2018).
Alter qualities of bolus for Brian such as the texture, taste, and temperature facilitate new sensations (Boop, Smith, & Kannenberg, 2017).

LTG
In 9 weeks, Brian will eat at the same time and table with family members, 2 out of 3 opportunities, for 7 consecutive days, as reported by caregiver for increased engagement in social participation.
STG
In 5 weeks, Brian will feed himself, using a pincer grip, with moderate assist, 2 out of 5 opportunities, for 7 consecutive days, as reported by caregiver for increased independence in feeding.

OCCUPATIONAL HISTORY
He was born at 34 weeks via emergency C-section, due to fetal heart arrhythmia.

He remained on a heart monitor with caffeine for one week, then without caffeine for one week.

1-year-old - Brian underwent cranial vault surgery to repair prematurely fused cranial suture (craniosynostosis).

Feeding (Sensory bins)
Grade (+)
Place large beads in a bowl or bin and have Brian pick up the mini dissovable chocolate chips to work on his pincer grip as well as promoting self-feeding. Using smaller chocolate chips requires higher skilled fine motor abilities. The larger beans will provide a new texture for Brian that he will have to navigate through in order to reach the preferred item, the mini-chocolate chip.

Spray whipped cream in a deep bowl or bin and have regular sized dissolvable chocolate chips dispersed throughout the whipped cream. The whipped cream can aid in Brian's texture exploration as well as providing an easier visual field to locate the chocolate chips. Chocolate chips are used to increase his engagement as well as promoting the pincer grip.
