About
Electronic health record is an electronic version of a patients medical history that is maintained by provider over time and may include data relevant to patients such as progress notes, past medical history and medications.
Basically, it is a digital version of a patient’s paper chart.
how does it works?
- The patient register at the reception at the hospital.
- Patients consult with doctor
- Doctor log on to the database system and search for patients details
- Patients tell doctors symptoms and problems, and analyzed by doctor.
- Doctors update the information in database
- The data is shared to the reception service of the hospital, under certain policy.

Authenticity: Give different access of information / Create username and password, however if passwords are simple, shared among users or never changed, the odds of accidentally allowing outside access to patient information could increase. Regular password reset or use complex password might reduce this problem
