Elderly males: with aging more estrogens are produced, which induces production of SHBG. SHBG has higher affinity for testosterone so therefore there'll be more free circulating estrogens than testosterone. This leads to an increase in testerone receptors on prostate cell and therefore making them extra sensitive to testerone. This causes Hyperplasia.

Bladder obstruction which leads to hypertrophy of bladder muscles, chronic cystitis( due to urine retention) & acute urine retention
Microscopic:
Nodules with excess prostate glands & stroma
Aggregates of dilated glands with numerous papillary folds
Glands lined with single layer of cells.
