I would use this form to document the behaviors, more so as evidence to support my observations of the child. This is also great as a second tier and not a first, because, even though this information may be accurate, by this age it is hard to tell whether there is really something wrong with a child or not. Also, many parents are in denial about there being something wrong with their child. So having both a behavioral chart and a functional behavioral checklist, plus any media or pictorial footage, would be great documentation/proof.
