NP Assessment
Categories
Memory e.g.. verbal, visual, working
Motor eg. speed, strength
Language eg. receptive, expressive, aphasia
Orientation eg. awareness of place, time, person
Emotional or psychological distress eg. depression, impulsivity
Sensation & Perception eg. visual acuity, taste, smell, tactile
Daily activities eg. toiling, dressing, eating
Visuospatial eg. construction, route finding, facial recognition
Attention eg. selective, sustained, shifting
Abstract reasoning & conceptualization eg. problem solving, EF
Steps (dynamic, flexible)
- Anamnesis (more structured & detailed): Diagnostic & Descriptive Questions
- Test Selection: standardized, individualized, composite
- Preparatory interview: purpose, nature, patient's capacity to take the test, administrative information
- Hypothesis Testing: maybe adding additional tests?
- Initial Planning: overall approach, first hypotheses, determine the purpose, when is it best to examine?
- Concluding: provide feedback, possible recommendations, treatment, report
-
Goals of NP assessment
identify mild disturbances eg. effects of head trauma, early symptoms of a degenerative disease
identify unusual brain organization e.g. in left handers or in people who had childhood brain injury --> valuable information to surgeons and can only be obtained from behavioral measures
-
-
promotes realistic outcomes: setting realistic goals, planning rehabilitation programs, assisting in understanding of the deficits
identify cerebral dysfunction & localize it where possible --> accurate, unbiased estimate of cognitive capacity
-
-
Issues
Test interpretation
-
-
-
-
-
quantitative data: "just a number", norms, baseline
qualitative data: objectivity issue, premorbid functioning
-
Testing procedure
-
in-between test intervals --> fatigue, memory effects
-
usage of technicians, assistants --> difficulties with interpretation
-
motivation, effort, attitude
-
-
-
-
feelings about the testing & its environment --> anxiety, frustration
-
-
Tests
Individualized
qualitative, require theoretical knowledge to administer & interpret, testing tailored to person's etiology & test performance, takes more time, Luria's approach
-
Standardized
fixed criteria, straightforward administration, scoring & interpretation
