Identify struggling students: check for noticeable inability to succeed and make sure there are valid reason. (check if it's not just mere laziness)
Signs may include: disinterest, anger, avoidance, constant failing, unable to communicate with peers, doesn't show any signs of improvement, fails to meet the basic expectations for that age group...
Gather visual/written proof. (take notes of behavior, gather test results, and if possible video of the student's activity during class. )
Look for ways to provide differentiation in classroom. Ask questions: what does the student need? Do they need more time? Better explanation? What type of questions are easier to comprehend for the students? Try to determine ways to help the student specifically.
Provide differentiation in classroom. Lessons and activities should be set and delivered in a way that is best fit for the student. By adding games, more visuals, simplified questions, grouping, or any other support teacher should implement what they have available.

Reach out to outside help such as teachers and other professionals that may be available to give good feedbacks or support.

If student's disability is covered under IDEA, or if the situation is beyond what the teacher can handle, reach out to other professionals for their support and opinions.
Teacher should communicate with the parents and possibly also contact the special education services.
