Large piece of foam fell from external tank which breached the spacecraft wing

The astronauts had spend 16 days in space
Hole on left wing resulted in atmospheric gases to bleed into the shuttle
Pressure readings lost from left side
Abnormal reading showed up at mission control
Mission control was unable to get a response back from the shuttle
Lost all temperature reading from sensors on the left wing
Why it Happened
Large piece of foam fell from the shuttle's external tank and breached the spacecraft wing
