Case Study 1: The One who is Always Off-Task
Jesse is a students of in my chemistry class. When I lecture or assign work, I see him doing other work, which I later found out it was homework for other classes or any homework or classwork I assigned previously. Jesse seems to have trouble organizing and managing his time.

Self-Monitoring form: Students are then held responisble of their behavior and also will help the student be more aware of their problem so they will be more proactive in solving it rather than relying on the teacher.

Daily planner and checklist for assignments:Since this student in particular has a problem with completing assignments then this will help in keeping check of what they have and help them become more organized and find out which assignments they missed and work on completing them or get the help needed. to complete them

Tier 1 General Interventions Tracking form
The forms helps the teacher monitor the students on a day to day basis and helps in showing what the teacher has done and what they can do next. It is appropriate for high school students since it is subtle so it would no t embarrassing the students , Also, high school students want to be treated like adults so in a way ,this mimics what jobs have.

Behavior contact form. This trackinng system also gets student involved. Also it helps the teacher and the students define the problem accurately and the list the steps the need to be taken. Having the document will help parties involved stay on the same page.
Daily Behavior/Assignment Log: same as the above mentioned systems. Daily logs remind the student to stay on task since he needs to fill out the log. This will also motivate him to stay on task and enforce the positive behavior.
Student Self Evaluation: The students is involved in tracking his behavior so he will know how well he progressed over the situation has been addressed
Case Study 2: The One Who Wants to be Alone
Shayna is a tenth grade students. Everyday, Shayna shows up five minutes late to my chemistry class. She does not like to work in groups so whenever we have group work, she ignores her peers and works individually and sometimes does not work at all. Her parents are concerned about her behavior since she has expressed that she does not want to go to school. They came to the school and asked for help to re-engage Shayna.

Description
Both of the systems listed involve the student which is appropriate for high school students since they like. to be treated as adults. These tracking forms will keep a record of students behavior and monitor progress which will help in deciding the. next course of action if the student fails to meet expectations

Since I am dealing with high School student, I like to involve in the intervention process since they much rather prefer that than just giving them a plan of what they should. Also, this holds them accountable for their actions and motivates to do better since they had some choice in the intervention,

At this point, the students has not responded to tier 1 and 2 interventions. This means that there might be underlying issues with the student that are causing this behavior. Therefore, referring to counsellor will help dig deeper into this problem. Using the behvavior tracking form will the help monitor the student progress after the involvement of the counsellor
