BONE & JOINT INFECTION
-
OSTEOMYELITIS
Suppurative males, 2-12yo
Femur
Epiphysis, Metaphysis
- vessels perforate the open growth plate
- break through the cortex to the subperiosteal region, spread underneath the periosteum → loss of blood supply → necrosis
- sequestrum = necrotic bone separated from viable bone by granulation tissue
- Involucrum = living bone in necrotic bone
-
- fat plane displacement = blurring of fat/muscle interface
- may see gas w/certain organisms
- moth-eaten pattern of bone destruction
- solid or laminated periosteal reaction
SPINAL
- Visceral infection, dental procedure, back pain
- Vascularity of disc allows for primary joint involvement --> loss of disc height
- Secondary endplate & vertebral body destruction, disc height loss
- Paraspinal line displacement from oedema/abscess
- paraspinal line displacement from edema/abscess
- epidural abscess and cord compression
-
Non-Suppurative TB, LBP
-
Tuberculous Arthritis - metaphysis spreads to joint
- changes with adjacent bone destruction
- Phemister’s triad = loss of joint space
- juxta-articular osteoporosis
- articular erosions
- fibrous ankylosiss
-
