cystitis
defination
Cystitis is inflammation and infection of the urinary bladder.
causes
The bacteria enter from the external genitourinary structures
risk factors
Tampon use
Inserting , changing ,or prolonged use of a urinary catheter.
Diaphragm for birth control .
Full bladder –if the bladder is not emptied completely it creates an environment for bacteria to multiply .
Sexual activity –sexual active women have the high risk of bacteria entering via the urethra
assessment and common findings
The patient may complain of frequency , incontinence , retention ,urgency ,hesitancy
Dark ,cloudy, or strong smelling urine ,pain just above the pubic bone in the lower back ,burning sensation when urinating
nursing management
Patient should drink a plenty of fluids if allowed to flush the toxins out .
Keep an intake and output record so that kidney function is monitored.
surgical management
Cysto-cystoplasty and cystolysis manipulate the innervation to reduce the bladders hypersensitivity ,considered for patients whose bladder capacity is normal
pathophysiology
the invading organisms gain entry through the urethra ,or the infection could be as a result of instrumentation with a catheter or a cystoscope.in women this can be due to an abnormal connection between the bladder and the vagina that occurs during childbirth ,or very rarely between the rectum or intestine .factors predisposing to urinary stasis in the bladder ,such as benign and malignant tumours ,strictures ,calculi and trauma may result in cystitis .the stagnant urine provides a conducive breeding place for organisms while it erodes the bladder mucosa .
health education
Practicing good hygiene after sex
Not postponing urinating
Avoid tight underwear and tight pants
Empty the bladder completely when urinating
Using neutral unperfumed soaps around genitals
medical management
Antibiotics are usually used for bacterial cystitis which are nitrofurantoin
