Self: Just like instructors say every class! "If you didn't write it down, it didn't happen!"
Use of radiographs, intraoral camera, etc can all help
Be sure to use language/phrases/vocab that other health care professionals can understand
1: Assessment
Gathering relevant information concerning the patient
Self: A comprehensive list of assessments are gone over with each patient in clinic, such as MHX, EOIO, Perio assessments (probing chart, gingival descriptions, calculus detection, etc), etc.
Patient observations: SUBJECTIVE
Measurement data: OBJECTIVE
Keep social, economical, and cultural considerations/adjustments in mind (e.g. health literacy or "dental IQ," social or economical background, work-related issues)
Dental Hygiene Diagnosis
NOT dental diagnosis, such as dentist stating cavity present, filling needed
Self: I think it will be difficult to train myself to say "suspicious lesion" instead of telling pt first thing that they have a cavity, etc.
Diagnosis helps us identify hygiene therapy needed, such as scaling, soft tissue needs (e.g. pathology), OHE, nutrition, etc.
Made by hygienist of existing/potential oral health issues
Planning
Dental hygiene care plan
MUST be gone over with patient prior to starting treatment, pt must consent or refuse
Self: We fill out comprehensive care plan for each patient in clinic, including unmet needs identification
MUST state goals/end result, cannot be indecisive
Can edit as treatment continues, but with pt's knowledge
Implementation
Provide OHE and nutrition counseling
Pain management (e.g. topical/LA)
Debridement procedures
Antimicrobial therapies (e.g. Arestin)
Chemotherapeutic agents
Coronal polishing
Fluoride
OHE on oral health responsibilities/home care
Tobacco cessation information
Follow-up/Post-op instructions
Proper documentation throughout
SELF: This is the treatment phase! Actually carrying out treatment.
Evaluation
Review effectiveness of therapies
Involve patient! Patients want to know. Example: Show pt lack of BOP with new perio charting.
Occurs after each visit or treatment
If unsuccessful, referrals may be necessary
SELF: We do this with plaque index, showing pt in mirror and on P.I. chart itself every visit in clinic
