Periop medicine ASA status
ASA score
Created to allow grouping of patients according to severity of comorbidities for statistical classification
Now used to guide periop risk as a number of trials have shown associated increase in periop risk with higher ASA grade
Grade
1 - Healthy patient with no systemic disease (Abs mort 0-0.3%)
2 - Mild or moderate systemic disease (Abs mort 0.3-1.4%)
3 - Severe systemic disease imposing functional limitation on patient (Abs mort 1.8-4.5%)
4 - Severe systemic disease which is a constant threat to life (Abs mort 7.8-25.9%)
5 - Moribund patient who is not expected to survive with or without the operation (Abs mort 9.4 - 57.8%)
6 - A brainstem dead patient whose organs are being removed for donor purposes
E = emergency case, threat to life or body part
Uses
Physical health status
Policy making
Performance evaluation
Audits
Resource allocation
Correlations
Operating times
Length of stay
Overall morbidity and mortality rates
Predictor of post operative outcome
Advantages
Simple, reproducible, easily learnt, not time consuming
Proven associations
Simple form of risk communication
Controveries
Doesnt consider age, sex, weight, disease severity, physical fitness, pregnancy or nature of planned surgery
Inconsistent application
Doesnt measure surgical complexity
Local disease not mentioned
Based on chronic disease, not acute and changing situation
Variability in including acute disease, smoking, pregnancy
Doesnt describe the general health of patient when excluding the condition that indicates surgery (e.g. ASA 1e patient post massive trauma)
