Try intervention strategies
Document the concern, as well as any interventions you have tried/ are trying. Record the date the intervention began.
Conference with student
Contact parent regarding concern. Student might have new diagnosis.
Refer to RTI Team
The team first looks at the system and how “all” the students are doing. The goal is to make sure that they system in place is reaching 80% of the students with Tier I instruction.
Include documentation of previous interventions
Refer to Multi-Disciplinary Team
Formally contact parents to get permission for testing or observation
Invite parents to meeting about student, including Principal or Administrator, Psychologist/Counselor, General Ed Teacher
The referral process includes a review of existing data about your student. Existing data includes, but is not limited to, any information you have provided, any medical information available, and any information about your student’s performance at school or in other settings (such as preschool, private school, at home, and other settings). Based on all of this information, the district must make a decision within 25 school days as to whether or not it will evaluate the student.

)
The student must have a disability or disabilities.
The student's disability/disabilities adversely affect educational performance.
The student’s unique needs cannot be addressed through education in general education classes alone – with or without individual accommodations and requires specially designed instruction (SDI).
Collaborate with Psychologist, Special Ed teacher, and other invested parties to ensure student's IEP is being carried out.
RTI Team will provide further intervention strategies
conduct formative assessments daily to adjust lessons to student's needs
small groups or 1 on 1 help allows for more direct instructions
know how a student learns best and adjust the lesson to fit this need (kinesthetic, visual, etc.)
