Chapter 19: Problem/Evidence Based Treatment Planning
Problem Based Learning
Technique for developing individualized treatment plans
Goals for periodontal therapy
Periodontal Therapy: Ongoing Care
Reflection: In clinic, we are taught to create a treatment plan based on each patients needs and assessment.
Problem List
Treatment Planning
Once exam and assessments are complete. Data that has been collected is used to create a treatment plan.
Reflection: In the clinic, we use the data collected through CAMBRA risk assessment along with Previser and Phillips assessment in order to determine patients likelihood for caries and periodontal disease
Risk assessment for periodontal disease
To determine the likely hood that the patient may develop periodontal disease in the future. All risk factors are taken into account for likelihood.
Patient Co-management and Referral to a Periodontist
Guidelines for Referral and Co-management
Patient who has one or more periodontal sites that show persistent sign of inflammation.
Phases of treatment:
Nonsurgical/Initial preperation
Periodontal Surgery
Restorative Care
Maintenance Care
Reflection: As a dental Hygienist we are responsible for nonsurgical and initial preparation. We must be able to identify when a patient has periodontal disease and if they need a referral to periodontist for surgery.
Treatment Guidelines
Periodontal Health
Oral prophylaxis treatment
Gingival Disease
Instrumentation and oral hygiene education
Periodontists
Chronic: Oral hygiene instructions and periodontal debridement. Surgical therapy if needed following debridement
Necrotizing Periodontal Disease
Referral to a periodontist may be needed for treatment.
Adjunctive Therapy:
Includes antimicrobials are often prescribed to patient to help combat pathogenic bacteria.
Refection: As a pharmacy technician, I had to filly many antibiotics prescribed by dentist and periodontist to fight oral infection
Geriatric Patients
Plaque Control
Age related factors may inhibit patients ability to perform adequate oral self care.
Xerostomia
Medication induced xerostomia in most elderly patients.
Reflection: My grandmother suffers from periodontitis due to her inability to perform adequate oral hygiene care.
