Sore throat (Acute)
Causes

Viral - rhinovirus, influenza, HSV1, EBV

Non-infective - allergy, GORD, drugs.
Diagnosis

Signs/symptoms - fever, tonsillar pain/exudate/pus/erythema. Malaise, nausea, headache, lymphadenopathy.
Management
Hospital referral
Comprimised airway/breathing 999.Clinical dehydration.
Peri-tonsillar abscess or cellulitis, parapharyngeal abscess, retropharyngeal abscess, or Lemierre syndrome.Signs of marked systemic illness or sepsis.
Primary care management
For people not in a vulnerable group, and without severe symptoms, or who have a FeverPAIN score of 2 or3 consider a delayed antibiotic prescribing strategy.
If antibiotics required :Prescribe phenoxymethylpenicillin as the first-choice antibiotic.
If there is a true penicillin allergy, clarithromycin is an alternative.
Adequate fluid should be taken during the course of the illness.
Ibuprofen and paracetamol can be used as an antipyretic and/or analgesic. Salt water gargling, medicated lozenges or anaesthetic sprays may provide temporary relief from throat pain.
Definition
Sore throat occurs when an acute upper respiratory tract infection affects the mucosa of the throat.
Clinical descriptions of acute sore throat include: Acute pharyngitis: inflammation of the part of the throat behind the soft palate (oropharynx).
Tonsillitis: inflammation of the tonsils.
