Value of case reports to anaesthetists
Levels of evidence
1: Systematic review of RCTs (Cochrane meta analysis)
2: At least one properly designed RCT
3: Other well designed study (Case control, cohort)
4: Evidence from descriptive studies (expert committees, based on clinical experience)
Case report
Detailed report of symptoms, signs, diagnosis, treatment and follow up of an individual patient (generally an unusual occurrence)
Level 4 evidence
Type of observational study
Pros
Important role in clinical education
High sensitivity for detecting novelty - very important for medical progress
Can be used to develop hypotheses for future research
Can alert anaesthetist to rare complications
Can accumulate into case series - which can add weight to observations and outcomes
Cons
No control group, therefore conclusions regarding efficacy of treatment are weak
Limited scientific validity (cf other methods)
Anecdotal evidence only
Prone to almost all types of bias
