Have a conversation with the student about his evaluation
If student is still struggling, meet with teachers and counselors for a 6-8 week plan
If there is insufficient or no progress, proceed to discussing with the Student Referral Team (SRT)
Discuss with the Team
Plan with the SRT or Student Referral Team composed of: Learning Support, Counselor, Admin
Arrive at a concensus about the Student Success Plan (SSP) after meeting the team
Discuss the SSP with the parents and the student
Seek approval from parents and collaborate together
Ensure that all teachers of the student are aligned with the plan
If no concensus is reached, rediscuss.
