Dental Hygiene Diagnosis
Analyzing the data collected during the assessment phase in order to identify oral health problems
Reflection: In processes of care, we learned to create a dental hygiene treatment plan using the dental hygiene diagnosis
Planning
Written dental hygiene intervention plan that is specific to each patient. Dental hygiene care plan uses data collected during the assessment phase to create a care plan for the patient.
Reflection: Dental hygiene treatment plan in our clinic is created prior to scaling and is reviewed with each patient and an instructor prior to beginning scaling,
Implementation
This phase includes the actual instrumentation and or intervention that is physically taken. The hygiene care plan dictates what is implemented.
Reflection: Scaling to completion would be an example of implementing a care plan.
Evaluation
This would be the evaluation of how patient responded to treatment.
Reflection: As a patient, I am often given an online survey to complete.
Documentation
Documentation is required for all treatment and services that were provided to the patient. This includes medical and dental history as well as all parts of the assessment and planning phase.
Reflection: In clinic we are to document each step that we take in our treatment plan and also document skill evaluations that were completed.
