Compartment Syndrome

Definition - A condition in which increased pressure within a limited space compromises the circulation and function of the tissues within that space
Venous outflow obstruction, further increased pressure
Encroaches on capillary pressure, microcirculation compromised, infarction
Result: Ischaemia, oedema and rhabdo

Medical
Fluid resus, attempt to maintain urine output (rhabdo, hyperkalaemia, myoglobinurea)

Clinical features
Pain out of proportion to clinical situation, pain on passive stretch
Pulselessness, pallour, paralysis, parasthesia
Diagnosis

Compartmental pressure monitoring (needle/cannula into compartment, fluid filled line, transducer) - Norm pressure 10-12mmHg. Fasciotomy if Compartment perfusion pressure <30mmHg (CPP = DBP - Comp pressure)
