- - - - Requires a local guideline that addresses patient selection, vascular access, volume of blood withdrawn, choice of replacement fluid, blood storage and handling and time of reinfusion
      - Reduction in transfusion requirements relies on the fact that there is reduced RBC loss intraoperatively
- - - - Continuation is reasonable in patients with high risk IHD or CVD
      - Discontinue if the bleeding risk outweighs the potential CV benefit e.g. neurosurgery
    - - Cease in patients undergoing elective orthopaedic surgery
      - Timing should reflect the agent's pharmacology
    - - Need to balance risk of life-threatening bleeding and thrombosis, particularly in patient's who have had a recent CVA, received a DES within the last 12 months or a BMS within the last 6 weeks
      - If decision to cease, this should occur one week preoperatively
