Tier 1: 30-year-old client was diagnosed as a child with autism but did not receive treatment. He has lived isolated from society for 5 years, having no face-to-face interaction for 3 years. Currently he lives in a community mental health facility, receiving mental health treatment. Autism: Incidence & Prevalence: Boys are 5x more likely, co-occurrence of 1 or more non-ASD developmental diagnosis: 83%, psychiatric diagnosis: 10%, Prevalence: 1:68 Symptoms: Difficulty in social interactions, developing friendships, challenges in interpreting body language/facial expressions, theory of mind limitations, limited or repetitive play routines, delays in basic language development, and echolalia. Prognosis: ASD is a lifelong disability with no cure (Atchison & Dirette, 2017).

Possible Co-Morbidity: Anxiety- Incidence & Prevalence: 40 million people diagnosed a year, generalized anxiety 6.8 million adults Symptoms: restlessness, tension, apprehension of danger, response to an unknown, vague, internal threat. Client expressed anxiety about re-integrating into society including living in the community, getting a job, and interacting with others socially (Atchison & Dirette, 2017).
Tier 5: Outcomes / Goals
Occupational Performance
In 3 months, the client will independently initiate and maintain a conversation with a peer at his weekly technology interest group meeting using eye contact for 50% of the conversation.
Well-Being
In 2 months, client will independently identify 3 educational interests he would like to pursue in an online learning format.
Participation
In 3 months, the client will demonstrate 5 essential job functions for a desired job independently.
Quality of Life
In 3 months, the client will understand and demonstrate basic hygiene tasks (brushing teeth, showering, using deodorant, and washing hands) independently.
Health and Wellness:
In 3 months, client will set up and manage his own medical appointments (dentist, GP, physiatrist, optometrist, and OT).
