Superficial partial thickness burns - heal spontaneously, require covering
Deep partial thickness burn - require excision and grafting
Full thickness burn - requires excision and grafting
4th degree - involvement of muscle, bone, other - require complete excision, leave limited function
Minor burns and conservative treatment
Superficial burns heal in 10-12 days by re-epithelialisation. Goals:
Prevent infection
Minimise pain
Timely wound healing, maintain moisture for healing, absorb exudate
Preserve physical function
Minimise cosmetic deformity
Burns dressings
Depends on stage of healing, guiding principles: facilitate healing, contour and adhere well, easy to apply and remove, allow full ROM)
Silver based products recommended for: contaminated/infected burns, deep/full thickness burns, minor burns with large surface area
Severe burns
First aid, initial resus and transfer to specialist burns unit
Treatment of extensive, full thickness burn wounds by early excision and grafting has been firmly linked to survival
Wound excision should be completed in the first week - before colonisation and neovascularisation
Skin grafts: autograft/allograft
