Anxiolytics, Antidepressants and Antipsychotics 1
Anxiety
Anti-depressants can be used to treat anxiety. SSRIs are used as the first line treatment for GAD but they have a 2-3 week lag phase. TCA are used for GAD and panic disorders. MAOI are used in resistant morbid anxiety with panic symptoms.
SSRI- Sertraline for the first 2 months in GAD and gradually increase the dose. If there is no improvement, then switch it to a different SSRI or SNRI (duloxetine/ venlafaxine). In extreme cases use BDZ as a short term treatment for 2-4 weeks
BNZ
Lorazepam, Nitrazapam, Diazapam

S.E- Confusion, impaired coordination, withdrawal symptoms
Beta blockers

To treat the somatic and physical symptoms such as tremors, sweating and shortness of breath
Buspirone

Does not cause sedation, motor incoordination and tolerance

S.E- Headache, nausea, restlessness, dizziness

Depression
SSRI
-
Fluoxetine, Paroxetine, Citalopram, Sertraline
Low toxicity in overdose and they are well-tolerated compared to TCA. Lack of addiction but will have withdrawal effect
S.E- Headaches, GI bleeding, sexual dysfunction, movement disorders. Risk of GI bleeding, hyponatraemia
Fluoxetine- long half life, safest in children

Serotonin Syndrome- Excessive central and peripheral serotonergic activity with autonomic dysfunction such as tachycardia, neuromuscular activity such as clonus and altered mental state such as confusion and mania
TCA
Amitriptyline, Imipramine

Irreversible MAOI

S.E- Headaches, hypotension, arrythmias

Phenelzine, Tranylcypromine and Isocarboxazid

Velafaxine- Inhibits presynaptic uptake of serotonin and noradrenaline and lacks the sedative and anti-muscarinic effects of TCA.
Mirtazapine- Pre-synaptic alpha 2 adrenoreceptor antagonist which increases the noradrenergic and serotonergic neurotransmission

Hypnotics
Z Drugs

Zalepon, Zolpidem tartrate, Zopiclone

Antihistamines- Diphenhydramine and Promethazine, Melatonin and Choral Hydrate (not receommended)
