MEDICAL DIAGNOSIS- GASTROENTERITIS
Nursing Assessment- 70 years old female, reports weakness and lethargy. She's been having liquid stools 2-6 times a day and has been vomiting at least 4 times a day. Altered LOC. Complain of pain or stomach cramping. Unable to tolerate liquids and or any kind of food due to vomiting and nausea.
Physical examination- Mild fever-38.9C
pulse- 90 BPM
respirations- 24/minute
scant urine output
b/p- 100/74
dry oral mucosa, cracked lips, decreased skin/tongue turgor
